VITILIGO 

UVB Therapy / commonly known as Phototherapy for WHITE PATCHES is a procedure in which a particular wavelength of UV lights is used to manage vitiligo lesions and to control its spread and recurrence. UVA and UVB are the two light sources which are used by Dermatologist to treat patients with vitiligo or leucoderma as they are quite effective in putting the patient to remission and control spread of vitiligo patches. UVB therapy is a quite effective way of treating the immune system or altered immune system which is affecting the skin or hair with vitiligo issue. UVA therapy needs psoralen which is given to the patient 2 hours before exposure to UVA lights. This makes the skin more receptive to UVA radiation and hence the patient needs to take care of skin to overexposure to sunlight after taking psoralen. Patients are advised to avoid sun exposures if possible for 8-10 hrs after ingestion of psoralen that has been considered for making UVA effective. Patient are advised to take treatment on alternate days. Even if it’s not possible to visit Dermatologist on alternate days, it should be ideally done 8-12 sessions in a month. On the contrary UVB Therapy does not require any medicine intake and thus the patient at will can take exposure to UVB under the guidance of Dermatologist as no prior intake of medication is needed prior to exposure. UVB therapy is gaining importance nowadays as it’s free of side effects and no blood investigations are needed because of psoralen intake. It’s a very safe treatment therapy and even can be used in pregnancy and children.

Vitiligo is a skin pigmentation disorder, Melanocytes are the cells that make pigmentation for skin and Hair. These cells are especially targeted by the immune system and resultant destruction gives rise to white patches / vitiligo . Vitiligo may follow a segmental pattern or in limited to particular area or may be widespread. Few patches may respond better to UVB therapy as compared to other areas and hence aim should be to prevent new patches from appearing and simultaneously treating old patches and UVB does the same. Hence UVB is a very good cost effective way to treat as well as to prevent spread of vitiligo patches. Once the hair at vitiligo sites become grey or white the response to the treatment becomes attenuated and hence the lesion should be targeted as soon as they appear or become evident. 

The exact cause of vitiligo is not known and various hypothesis and postulates are given like production of antibodies against Melanocytes, emotional distress, hereditary pattern, neural theory are the important one. Vitiligo is often seen associated with some auto immune disorders like Alopecia Aerata, Hyper or hypothyroidism, insufficiency of adrenal glands, pernicious Anaemia are worth mentioning. Vitiligo /Leucoderma mainly is a Esthetic issue if other associated disorders are ruled out. Some vitiligo patches may be quite resistant to treat such as palm and soles, and vitiligo in non Hair bearing areas  or acral or digital areas. Vitiligo patches may show regain of colour which is evident as small dots of pigmentation in the center /periphery of the lesion which later on join, or circumferential Reduction in the vitiligo patch. Sometimes ointments for local application are given by treating dermatologist to enhance recovery and to prevent relapse or for Prevention of new lesions.
